    Assuming Opposing Force's nuke is canon (Marc Laidlaw said it was, anyway), the entire facility would have been destroyed. The instructions on the bomb are pretty clear about its blast radius:

    MANUAL INSTRUCTIONS FOR A: MARK IV THERMONUCLEAR DEVICE

    This device carries a GRADE C plutonium control detonator.

    With this device an approximate safe distance of 55 kilometers is recommended.

    STEP1: Indispose the gravatronic rev limiter to 11.

    STEP2: Rotate red knob to the on position.

    STEP3: Press button labeled B.

    A minimum safe distance of 55 kilometers is pretty similar to the Tsar Bomba. In fact, it's about 4 km larger. Assuming the "Thermal Radiation Radius" of 51.4 km is considered the minimum safe distance, then the Mark IV Thermonuclear Device is possibly the most powerful nuke ever created, at least in the Half-Life universe. Since it's a tad stronger than the Tsar Bomba, it'd have to be a bomb with over 50 Megatons of explosive power.

    Thus, assuming Black Mesa isn't larger than Albuquerque, the entire facility would be leveled, and it's unlikely there would be any nostalgic items there if we return in a later game (aside from the radioactive waste, that is.) At the very least, a little over 113 km2 of the facility would be vaporized instantly. Thanks for coming to my Ted Talk. Hopefully this is interesting enough for ye guys. Thanks to /u/White_star_lover for telling me the nuke's minimum safe distance, by the way.
